Harpeth Pediatrics Vaccine Schedule 2 months - Pentacel (DTaP, IPV, HIB), Hepatitis B, Prevnar, RotaTeq 4 months - Pentacel, Prevnar, RotaTeq 6 months - Pentacel, Hepatitis B, Prevnar, RotaTeq 9 months - None, unless catch up shots need to be given 12 months - Prevnar, Varicella, Hepatitis A 15 months - Daptacel (DTaP), MMR, a fourth dose if HIB will be given at this visit when the shortage is over 18 months - None, unless catch up shots need to be given 24 months - Vaqta (Hep A) 4 years - You have the option to split up the kindergarten shots and give DTaP and Varivax at this visit 5 years - Daptacel (DTaP), IPV (Polio), MMR, and Varivax or just MMR and IPV if your child is on the split schedule 11 years - Adacel (Tdap), Vaqta (Hep A) and/or Varicella (2 doses given 1 month apart) for children not already immunized, one booster dose for children who did not already receive a second dose 12 years - Menactra, Vaqta (Hep A) Additional Immunization Information: |
